FSD have recently adopted a new reporting procedure to address and reduce near miss incidents on site. All sites are being supplied with freepost postcards, which allow personnel to send in their concerns and details of a near miss directly.

Serious events are addressed with the Site Management to immediate corrective measures, however, the data gathered will also be entered onto a reporting system and analysed to provide useful management information. It will also allow us to produce trend reports to identify where weaknesses are occurring, to implement effective corrective actions to prevent recurrence, and to recommend global actions, so that benefit is not just localised to one site.
